  • If you would like to be present and more than one event, you must complete each form for the events you would like to be at.

Unity in Action is not responsible for any damages, lost or stolen items. Food Vendors will receive an email with further instructions for the event.

The food vendor fee is $260.00; this includes a $45.00 City sales license fee.

  1. Provide your sales tax permit as required in Section 77-2705
  2. Provide a certificate of insurance or public liability bond for not less than $300,000.00 for property damage and injuries, including injury resulting in death, caused by the operation of the vending business, (with City listed as certificate holder).
  3. Provide and maintain a permit (or a certificate of health inspection) from the Nebraska Department of Health, if engaged in the sale of prepared food or beverages.
  • Setup & operating at venue by 6pm
    • Venue instructions will follow via email
